What is 'barcode'?

• Cannot register barcodes that are not from iSearchme or have already been used
• Only one barcode can be entered per person.

Fill out the consent form and attach barcodes

• Please check the contents of the enclosed test kit and fill out the consent form.
• Please attach the barcode to the upper right corner of the consent form.

Registering barcode

• With smartphone app, you can go to registration page by typing the address or scanning QR
• Please register your consent form according to the procedure.
